Request Forms

Ad Hoc Project Request Format

CPHST Ad Hoc Projects may be requested online at http://adhoc.cphst.org. State Plant Health Directors (SPHDs), Regional and Riverdale Program Managers, Assistant Directors, and PPQ Executive Team members may apply for projects directly through the Ad Hoc website. Descriptions of the information the requestor will need to complete an ad hoc application is listed below. Please note that this is not the actual form and should not be used as such. Only online submissions are accepted.

Project Title: An informative and succinct description of the project. Please include enough information so that readers get a general understanding of the project, but avoid lengthy and overly-detailed descriptions.

Project Description: An explanatory summary of the problem or issue, project purpose, its significance, relative importance, justification, and approach.

Deliverables (1000 Character Limit): Complete usable items distributed to the requestor(s) at the conclusion of the project which provide scientific support to CPHST stakeholders. Deliverables include items such as the finished product, completed processes, written protocols, etc., that you, the client need to solve your scientific problem. All project applications should be developed with clearly thought out deliverables in mind.

Financial Savings (1000 Character Limit): Estimation, in dollars, of cost saving the deliverables could provide to PPQ or other stakeholders.

Time Savings (1000 Character Limit): Estimation, in time, of how the deliverables can reduce hours of work or procedures.

Other Savings (1000 Character Limit) : How the deliverables will make PPQ more effective/efficient in dealing with specific problems, i.e. prevent market loss, support trade, pest prevention and management.

Relevant Background Information & Publications (1000 Character Limit): Information pertaining to the completion of the project, reference information, and other collaborators.

Note: Ad Hoc project submissions are only received and approved by designated users through the official Ad Hoc URL. Please do not mail, fax, or email proposals outside of this designated system, as they will not be processed.

Submissions

How do I edit my submission?

There is not an edit function in this program. Once you click Submit the project proposal is sent via email to all appropriate parties, i.e. your supervisor (ARD, AD) and CPHST Associate Director. To minimize grammatical errors and to create a well thought out proposal, you are encouraged to first write the proposal in a word processor and then copy and paste the information into the respective sections of the form. You can use the preview button on the request page to look over your request before submitting.

Minor additions and corrections to clarify the submission can be attached in the comment field once a project has been submitted but minor errors (i.e. spelling, grammar) will remain. Any major edits will have to wait for your Associate Director, Associate Regional Director or CPHST Associate Director to request revisions.

My session ended while I was composing my request. Is it gone?

More than likely it is gone, especially if you selected cancel in the pop up box. Solution: If you think you will be idle for more than 30 minutes while composing your request, it is recommended that you complete the sections in a word processor such as MS Word. Then when you have finalized your project request you can copy and paste the information into the respective sections. Please keep in mind the character limits for each section if you take this approach.
